Alarm Management

Alarm fatigue as a documented patient-safety issue, physiological vs. technical alarms, and the real role a BMET plays in managing it.

Alarm fatigue - clinical staff becoming desensitized to frequent, often clinically-insignificant alarms - is a documented, real patient-safety issue. It has been a recurring Joint Commission National Patient Safety Goal, driven by real incident reports where a genuinely critical alarm was missed or delayed amid a background of frequent nuisance alarms.

Physiological vs. technical alarms

A physiological alarm means something is wrong with the patient (a heart rate outside set limits, a desaturation event). A technical alarm means something is wrong with the device or its connection to the patient (a lead-off condition, a sensor disconnect, a low battery). These need genuinely different responses, and troubleshooting one as if it were the other wastes time during an actual emergency - a technical "lead off" alarm that gets treated as a physiological event delays the real assessment the patient needs, while a physiological alarm dismissed as "probably just a sensor issue" delays real intervention.

A BMET's role

  • Verify alarm limits are set appropriately for the patient population during PM and commissioning, not just left at factory default - a neonatal unit and an adult ICU need meaningfully different default limit sets, and a monitor commissioned with the wrong population profile will either under-alarm or generate excessive nuisance alarms from day one.
  • Never permanently disable an alarm to work around a nuisance condition - fix the underlying cause (a poorly-adhered SpO2 sensor causing false desaturation alarms, for example) instead of silencing the symptom.
  • When a unit or department reports chronic alarm fatigue, that's a system-level signal worth escalating to a formal alarm-management review - sensor placement training, limit customization by unit, or a technology change (e.g. a motion-artifact-resistant SpO2 algorithm) - rather than something any single repair ticket can fix.
