Standards & Regulatory

IEC 60601, AAMI, FDA classification and clearance pathways, Joint Commission, and ISO 13485 - what each one actually governs and how they interact.

The regulatory landscape around medical equipment involves several distinct bodies with different scopes, and confusing them is a common source of miscommunication with auditors, vendors, and clinical leadership.

IEC 60601 series

The international baseline for medical electrical equipment safety and essential performance. -1 is the general standard every device must meet; -1-x collateral standards add requirements for specific hazard categories (e.g. -1-2 for electromagnetic compatibility, -1-8 for alarm systems); -2-x particular standards add requirements specific to one device type (e.g. -2-12 for critical care ventilators, -2-19 for infant incubators). A device is compliant with the general standard plus whichever collateral and particular standards apply to it - not just "60601 compliant" as a single blanket claim.

AAMI

The Association for the Advancement of Medical Instrumentation publishes the US-harmonized versions of IEC 60601 (as ANSI/AAMI ES60601-1) plus its own guidance documents covering practice areas the international standard doesn't fully address - ST79 for steam sterilization, ST91 for flexible endoscope reprocessing, and EQ56 for medical equipment management program guidance. AAMI standards are frequently referenced directly in accreditation surveys even though they're technically voluntary consensus standards, not federal law.

FDA: classification and clearance

The FDA assigns each device type a class - Class I (lowest risk, general controls, e.g. bandages), Class II (moderate risk, requires special controls and usually 510(k) premarket clearance, e.g. infusion pumps), and Class III (highest risk, requires full premarket approval (PMA), e.g. implantable pacemakers). Most equipment on this site reached the market via 510(k) clearance, which demonstrates substantial equivalence to an already-legally-marketed predicate device rather than requiring a full new clinical trial. Joint Commission (or equivalent accreditor)

Sets the accreditation requirements for a facility's medical equipment management program as a whole - PM inclusion criteria, incident-investigation obligations, and environment-of-care standards. Accreditation is what a facility needs to participate in Medicare/Medicaid reimbursement in the US, which is why survey findings carry real operational weight beyond just "best practice."

ISO 13485

The quality-management-system standard manufacturers are certified against, covering design controls, production, and post-market surveillance. It's relevant to a BMET mainly when evaluating a manufacturer's own service documentation and change-control practices - a manufacturer with a mature ISO 13485 QMS tends to produce more traceable, better-documented service bulletins and field corrections.
